And that’s good advice. Maintaining muscle strength is an important part of staying active, mobile, and independent as you age. But there’s another question that may be just as important: Are you strengthening the right muscles—and are those muscles actually doing the work? This distinction is especially interesting when we look at the muscles around your hips. Your gluteus medius and gluteus minimus help stabilize your pelvis and control your body during walking, stepping, and other movements. Research suggests that greater hip-abductor strength is associated with better performance on balance and mobility tasks. But strength is only part of the story. How your nervous system activates your muscles and how well your muscles work together also matter. That’s why simply doing more exercises isn’t always the answer. Sometimes, the key is learning to feel the right muscle working, control the movement, and progressively build its capacity. Your Hips Do More Than Move Your Legs Your hips are constantly working to help control your body. Consider walking. Every time you take a step, your body briefly has to balance over one leg. Your hip muscles help stabilize your pelvis and control the position of your leg. Now consider stepping over a curb. Or walking around someone in a grocery store. Or stepping sideways to avoid an obstacle. Or catching yourself when you lose your balance. Your hips have to respond. This requires more than simply having strong muscles. Your muscles need to be able to activate, produce force, and coordinate that force with the rest of your body. What the Research Says About Hip Strength A systematic review examined 59 studies involving 2,144 participants to investigate the relationship between hip muscle strength, muscle activation, muscle structure, balance, and mobility. One of the primary findings was that greater hip-abduction torque capacity was generally associated with better performance on balance and mobility tasks. These tasks included measures such as: Walking Stair climbing Timed Up and Go Balance testing The researchers found this relationship across young and older adults, although they noted that there was considerably less research available for middle-aged adults. The authors concluded that greater hip-abduction torque capacity appears to be important during balance and mobility tasks across age groups. That’s a meaningful finding. But there’s more to the story. Your Muscles Need to Know When to Turn On Your muscles don’t simply sit there waiting to become strong. Your nervous system constantly determines which muscles need to activate, how much force they need to produce, and when they need to do it. Think about stepping off a curb. You don’t consciously tell your gluteus medius: “Okay, activate now.” Your nervous system handles that automatically. This ability to activate a muscle appropriately is called neuromuscular activation. And research suggests that neuromuscular activation is an important component of balance and mobility. The systematic review found that hip-abductor activation varied depending on the task and the age of the individual. In some balance and stepping situations, older adults activated their hip abductors later than younger adults. This gives us an important insight: Having a muscle doesn’t necessarily mean you’re using it effectively. Can You Actually Feel the Muscle You’re Trying to Strengthen? This is where our approach at PhysioFit of NC becomes particularly relevant. Imagine you’re performing a hip-strengthening exercise. You complete 15 repetitions. But instead of feeling the outside of your hip working, you feel it primarily in your low back, front of your hip, or another area. You technically completed the exercise. But did you accomplish what you intended? This is where “Feel it to Heal it” comes into play. We want people to develop a better connection between their brain and the muscles they’re trying to train. Can you feel the intended muscle? Can you activate it without excessive compensation? Can you control the movement? Can you progressively increase the challenge? These questions can be more meaningful than simply counting repetitions. Strengthening a Muscle Is Different From Learning to Use It Let’s say you want to improve your gluteus medius. You could simply perform more hip-abduction exercises. But there’s another approach. First, determine whether you can actually activate the muscle. Then learn to feel the muscle working. Then improve your ability to control the movement. Then progressively challenge the muscle. Then incorporate that strength into functional activities. The goal is to turn an isolated exercise into something your body can actually use. Why Compensation Matters Your body is incredibly adaptable. If one muscle isn’t doing its job effectively, another muscle may step in to help. That’s useful. It’s one of the reasons we’re able to continue moving even when something isn’t functioning optimally. But compensation can also make it difficult to target a specific muscle. For example, someone performing a hip exercise may use their low back or tensor fasciae latae more than intended. The movement still happens. But the gluteus medius may not be receiving the same training stimulus you were hoping to create. This is why movement quality matters. The goal isn’t simply: “Did you complete the exercise?” The goal is: “Did you perform it in a way that trains what you’re trying to improve?” What About Fat Inside the Muscle? One of the particularly interesting findings in the systematic review involved muscle structure. As we age, muscle composition can change. One of those changes can involve increasing amounts of fat within and around muscle tissue, sometimes referred to as fat infiltration or myosteatosis. The researchers found limited evidence suggesting that the amount of fat within muscle may be an important factor related to balance across age groups. However, they emphasized that the evidence is still limited and that more research is needed. This is important because muscle quality isn’t simply about how large a muscle is. Two muscles of the same size may not have the same functional capacity. Muscle Quality Matters Think about muscle as having two dimensions: Quantity How much muscle you have. Quality How effectively that muscle can produce and control force. As we age, maintaining muscle quality becomes increasingly important. And this is one reason why simply looking at muscle size doesn’t tell the whole story. Can Exercise Help Maintain Muscle Quality? Regular physical activity and resistance training are important tools for maintaining muscle strength and function as we age. Resistance training provides a stimulus that tells your body: “We still need this muscle.” Over time, appropriately progressed resistance exercise can help maintain or improve strength and physical function. But there’s an important distinction. You don’t want to simply move more. You want to move with purpose. If your goal is to strengthen a particular muscle, you want to make sure you’re appropriately challenging that muscle. That’s where assessment and movement awareness can become valuable. Don’t Wait Until You Lose Function One of the challenges with aging is that physical changes can happen gradually. You may not notice that your hip strength has declined. You may not realize that you’re using one side more than the other. You may not notice that your balance has changed. You may simply start avoiding things. Maybe you stop getting on the floor. Maybe you park closer to the store. Maybe you stop carrying heavy groceries. Maybe you avoid stairs. Maybe you stop gardening. Maybe you stop strength training because certain movements don’t feel right. These changes may seem insignificant individually. But over time, they can reduce your opportunities to maintain physical capacity. The Goal Is to Keep Using Your Body Your body adapts to what you ask it to do. If you consistently challenge your muscles, they have a reason to remain capable. If you stop challenging them, your capacity can decline. That’s why the goal of healthy aging isn’t to avoid movement. It’s the opposite. The goal is to maintain your ability and confidence to keep moving. And sometimes that starts by figuring out what’s preventing you from moving well.
